Get the complete picture on patients with Medicare

Data at the Point of Care (DPC) is a secure application programming interface (API) that shares Medicare claims data with providers. Centers for Medicare & Medicaid Services (CMS) offers DPC to support informed, coordinated care for people with Medicare.

DPC data in action

Personalized care

Thanks to DPC, Sarah, a primary care doctor, has a more complete picture of her patients with Medicare. She sees Medicare claims data in the electronic health record system (EHR) she uses every day. This means less time looking for missing details and more time working with her patients and making informed care decisions.

Managing chronic conditions

Naomi has Type 1 diabetes and recently moved to a new city. Thanks to DPC, her new doctor sees Medicare claims data from different providers in his clinic's EHR system. He identifies which screenings and exams Naomi already received. Instead of piecing together Naomi's past history, he can focus on managing her care needs today.

Provider analytics

Elena, a diabetes care coordinator, manages care for dozens of people with Medicare who have chronic conditions. Medicare claims data from DPC informs her about clinic visits, hospital admissions, procedures, and medication refills. A better picture of her patient population helps her identify gaps, perform outreach, and get patients the care they need.

Integrate with EHR systems

DPC shares Medicare Parts A, B, and D claims data in the HL7® FHIR® standard. This format integrates with EHR systems to display Medicare data in your existing workflows.

Coordinate across care teams

Medicare claims data bridges information gaps across provider networks, supporting coordinated care and smoother transitions between care settings.

Relieve burden for people with Medicare

When patients don't have to repeatedly explain their health histories, providers can spend more time on treatment, education, and answering questions. Patients benefit from more informed, coordinated care and more meaningful conversations with their care teams.
